This review examines the role of neonatal Fc receptor (FcRn) inhibition as a novel targeted strategy for treating chronic inflammatory demyelinating polyradiculoneuropathy (CIDP), an immune-mediated disorder that causes progressive weakness and sensory loss. While standard therapies like immunoglobulins, corticosteroids, and plasma exchange effectively manage symptoms for many, they are limited by non-responsiveness, persistent residual deficits, side effects, and administration logistics. By clearing pathogenic IgG antibodies, FcRn inhibitors address these unmet needs, highlighted by the Phase 3 clinical success and subsequent approval of efgartigimod.
The authors synthesize current pathobiology and clinical trial data to provide practical guidance on integrating approved and emerging FcRn inhibitors into real-world CIDP management, noting that while highly effective, treatment response remains variable across the patient population.
